The following descriptions represent standard monthly reports that CCC provides
clients. The content of these reports enable effective and efficient management of
the administrative process, plus they provide built-in security features for liability
protection. Furthermore, if additional reports are needed, we have the capabilities to
create reports to meet demand.
Department of Labor Notice Report
This report provides a list of employees and dependents that were mailed a General
(Initial) Notice of COBRA Rights and Responsibilities. (Employees and Dependents are
provided this notice when their coverage under a group health plan commences.)
Principal Qualified Beneficiary Report (Participant Status Report)
This report provides a list of principal beneficiaries (individuals) that
experienced a loss of group health coverage because of a Qualifying Event. The report
indicates that all eligible beneficiaries were mailed the appropriate eligibility,
election, and enrollment materials. In addition, the beneficiaries were provided with
a certificate of coverage required by HIPAA.
Payment Voucher Report
This report provides a list of COBRA premiums received. The report identifies the
name of the COBRA participant, the type of coverage paid for, the premium due date,
the premium payment date, and the premium payment amount. In addition, the total
premiums received during the month and the 2% administration fee is included. |
